Mental Health
Program
The Mental Health Program is designed to support individuals living with serious and persistent mental illness to achieve and maintain a quality of life that suits that particular person. The program assists people to develop the skills and supports necessary to maintain a level of independence that allows them to live and thrive in the community. This is done with a client-centred approach. Support can be provided in a community setting or within the person’s home environment. The program also offers opportunities for social, recreational, vocational and group support, providing people with the chance to meet and develop natural and alternate community supports.
The fundamental principles and values of the Mental Health Program are to use a flexible and client-centred approach to increase the person’s success and satisfaction with his or her living, learning, social and/or work environments. This is done in the least intrusive manner agreed upon by worker and client.
The aim of the program is to provide people with services and support that will meet their individual needs and that will increase their quality of life in the community.
Dual Diagnosis Program
The Dual Diagnosis Program, is a flexible 24 hour support program for individuals who live with a serious mental illness and a developmental disability (dual-diagnosed). The program assists individuals to develop the skills and community supports necessary to maintain a level of independence. Support is provided to individuals within a community setting as well as within individual's home environments. The program also offers social, recreational, vocational and group support through which clients are provided with opportunities to develop natural community linkages.
The program operates in 2 support settings: a 24-hour staffed apartment complex consisting of 6 apartment units and in Supported Independent Living arrangements consisting of individuals who live in the community in market rent and/or subsidized housing. At present 6 individuals are receiving intensive support in the apartment complex and an additional 29 individuals are supported by CSW's in the community.
The supports offered are varied to meet the needs of each unique individual. Based on the concept of PERSON CENTERED PLANNING, (Annual Planning) support plans have been individualized to the wants, needs and requirements of each individual supported. Various assessments are completed throughout the year to track the progress of each individual. The supports provided are:
Case management
Monitoring of medical needs
Development and monitoring of learned goals and objectives
Assistance with daily living activities (within home and community)
Assistance with mental wellness
Assistance in coping with symptoms of the illness(s)
Developing and maintaining natural linkages
